A geriatric care manager specializes in assisting older people and their families with long-​term care arrangements. RN Care Managers are experts in disease processes, medication management and behavior disorders. Our quality caregivers are oriented and monitored by an RN care manager who provides and implements the plan of care. They can help you maintain your independence and live in your home, if you can. Our nurses will coordinate care when you or a loved one is being discharged from a hospital or nursing home.

History

Established in 1997.

Colleen Van Horn started Innovative Healthcare Consultants while working in a hospital for many years and seeing elderly patients leave the hospital to return in a few days because they couldn’t manage their care alone at home or they needed someone to monitor their progress. Innovative Healthcare Consultants has clients throughout San Diego County and into southern Riverside County.

Colleen has been a RN for more than thirty years. In 2013 she was the President of the Western Chapter of the National Professional Geriatric Care Managers. She and all the RN’s and caregivers at Innovative are dedicated to making life safer and more secure for the elder person.
