I find it odd they want your name, address, DOB, emergency contact and even social security number BEFORE you can make an appointment. Social security?!

Every time I have an appointment you are running 60 – 90 minutes behind. Please give your patients a courtesy call or fix your scheduling. It’s rude. People do not have time to sit around because you’re always running late. This is the 4th time it’s happened in a row.
