St. Francis Pavilion

A patient of mine was in St Francis Pavilion awaiting surgery with a very painful condition. Twice during a short stay the nurses failed to give her pain medication because of communication snafus which I was able to resolve within minutes (maybe seconds) of being advised of the problem by my poor patient who was suffering in pain. The fact that they had neither the creativity nor integrity and ingenuity to resolve this simple matter on two separate occasions is unforgivable.

I call this place Ramshackle Estates. PT staff is good, nurses… Almost all CNAs, with maybe 1 to 2 RNs per shift. Not the worst but definitely not the best.

The staff here is rude rude rude and ABUSIVE!!! THIS PLACE HAS ELSER ABUSE WRITTEN ALL OVER IT. They even tell you that you should take the patients close home because they will lose it. SO GUESS WHAT! I told the staff that I would wash the clothes AND THEY LOST IT ANYWAYS!!! I give them one star because the therapist was really gentle with my dad!!! The staff abusers the patients when no one else is around I saw one of the CNA’s hit a patient on the hand!!!

My uncle is 92 yrs old and was a patient at St. Francis Pavilion in Daly City for six weeks recently. He’d had a fall and needed physical therapy and skilled nursing care after being discharged from the hospital. The daily care he received from the nursing staff under the direction of Lina and the physical therapy staff under Arvin was excellent, and the communication between their social worker Dinno and our family was outstanding. We were kept informed of his progress regularly, and in our opinion my uncle could not have received better care. The occupational therapy program was also helpful, and Gloria in customer service was always available to answer questions and help coordinate things. Everyone including the receptionists and administrative staff to the dietitian was friendly and approachable. The one area of improvement could be to offer more variety in the meals, but it’s hard to find fault with anything at this skilled nursing center.

The staff is polite and helpful, my grandfather is a patient there and they’ve been taking care of him with no issues thus far and they have been attentive and accommodating of his needs. They’ve also done a great job of keeping me informed with periodic updates as well as notifying me of any changes or serious issues.
There’s also parking as well as bus routes nearby to and from the Colma BART station.

There are negative reviews here about this facility but I am speaking from my own experience with St. Francis Pavilion and I am not addressing nor refuting anyone else’s reviews and comments here on Unilocal.
My mom stayed at St. Francis Pavilion from Feb to mid-​May 2015, and passed away due to advanced dementia on May 21.
The whole time my mom was at St. Francis, I found their staff very professional, most especially the staff at Station One East, the Customer Service Rep, Gloria and the Social Services staff, Dinno.
I have no negative things to say about them — they all have been very professional, dedicated and very caring and always addressed our concerns right away, without making us feel like we were complaining and making life more difficult for them.
In the last 2 months my mom was in hospice, all of them made sure my mom was comfortable, which is the main goal of hospice.
I salute the staff of St. Francis Pavilion for the excellent care they gave my mom, and their dedication in caring for patients in their facility.

My grandmother was admitted due to femur fracture and received rehab services from this facility. The rehab department helped my grandma’s mobility with great and achievable goals. Most of the staff was friendly and helpful to my grandma and my family. Staff of saint Francis pavilion provided great customer service. The facility’s environment was not bad at all. My grandma is discharged from this facility already and she is extremely greatful to the staff of saint Francis pavilion. My grandma is 84 years old and now able to walk after rehab services. Thanks to all staff of saint Francis!!!

I was a patient here and they provided me with the utmost care to get me back on my feet through a car accident & a stroke. Had Physical Therapy, Occupational Therapy, and speech therapy. I would not be where I am right now without the 2 – 3 months I spent here. All the nurses and CNAs always greeted me with a smile which made my stay here comfortable and put a smile on my face. There physical therapy was rigorous but it ultimately helped me, got me back on my feet and was able to walk with a walker like this gave me hope as soon as I was on my two feet. Even speech therapy like I was able to think a lot clearer and have really good conversations with people. Everything I experienced here was the biggest stepping stone back into moving forward with my future!

Both my grandparents had their rehab and stayed at St. Francis Pavilion. They stayed at different times but the care they received was consistent and good quality. They also made a lot of friends with the great staff there that made them feel at home. I can recommend this skilled nursing facility as a good home away from home. Nursing care and rehab services were delivered well and management was very helpful. Visiting hours are also lenient even if you need to visit at late hours.

THIS FIVE STAR RATING IS FOR THE REHABILITATION DEPARTMENT ONLY. I have been a patient here at the St. Francis Pavilion for a tad over six months and received both Physical Therapy and Occupational Therapy in their Rehab Department. I give them the highest rating because of their total professionalism in tailoring their program to the individual patient … and I am a stubborn and cautious one at that. The staff encouraged me to do the best I can within my narrow comfort zone and excelled in the Physical Therapy part with ease. In the Occupational Therapy phase, they worked with me at my own personal levels. The staff never was judgmental but truly professional in all aspects at all times. They know whom they are so no need to list names, but I am truly grateful for them and their skills. They are each very special to me. I am getting out this time next week and will carry these memories forever.

My grandmother (stroke patient) just passed here recently in the care of these STUPID PRICKS. The dietician who gave a 5 star review needs a BIG reality check:
— FOOD: everything you’ve read in the other reviews about trays just being dumped there are absolutely true. My grandmother suffered a stroke and came to this hell hole, so obviously she can’t feed herself. We visited VERY OFTEN and around mealtimes, we would just see her tray sit there at the FOOT of her bed. How was my 80 something year old grandmother who weighed 70 pounds (tiny woman) supposed to even get to the food? This tray would just sit there for the longest time — 45 minutes to an hour. SHE COULDN’T EVEN TALK. Meanwhile these so-​called «hardworking compassionate people» as described by the dietician are just standing around and talking to other patients or talking to each other. VERY common occurrence. «Understaffed» means «not doing much» in this case. Another example: plenty of times I saw a male employee just sitting in his chair looking at his phone while he’s supposed to be «watching over» another patient in the same room!
— CARE: Poor care. i.e., my uncle went to see my grandmother one afternoon and noticed a puddle by the side of her bed. He pulls back her blankets and finds that she had peed and it soaked through her sheets and leaked onto the floor. The excuse we heard was «oh, she peed a LOT, I just checked her 2 hours ago.» DOES THIS MAKE SENSE? She peed SO MUCH in 2 hours that it leaked onto the floor and created a giant puddle? Sounds like «I haven’t checked up on your grandmother in more than 4 hours and I don’t have the balls to own up to my mistake so I’ll just lie to you.»
Can you complain? Of course you can, but will they listen? No, they’ll just feed you with BS excuses and empty promises. In the end, they only want money.
I’ll spare you the other horror stories. If you want your loved ones to starve and sit in their own pee, by all means send them here. Otherwise, I would suggest sending them somewhere else.

Alex is a great and wonderful director of admissions to work with. He was extremely pleasant and answered all my questions with his full attention. I generally don’t get great feedback from people working in admissions, but Alex was great!

My wife Erin has been at St. Francis since her hip surgery. They have been great, taking care of all her needs; managing her pain, physical therapy. The staff is welcoming and always up beat and smiling. My wife gets out by the end of the week and is very happy with her stay. We both would strongly recommend St. Francis Pavilion to anyone of our friends and family. Thank you :)

Terrible Place my father had a small bed sore when he went there it was a level 1 the Saint Francis staff left him on it for three weeks where it grew to a stage four it is 2 and a half inches in diameter .He also contracted a nasty intestinal infection transmitted threw feces then they discharged him to a Board & Care he is back in the hospital where they surgically removed the dead skin now he has a hole that is 1 inch deep its sad that this place is open for business.

VERY short-​staffed!!! Takes a loooooong time for staffs to come after requesting help.
My grandma was admitted here after her hip surgery. They were supposed to turn her one side to another every 2−3hours, but of course it wasn’t done, it was probably done only twice a day. As a result, my grandma developed a deep, quarter-​sized, painful bedsore within roughly a 2−½ week stay here. She was later admitted to Seton due to other concern, even the nurse and PT at Seton were surprised to see such a deep ulcer within such a short stay!
Also, it seems to me the staffs responded faster to the other Tagalog-​speaking roommate as compared to the other non-​Tagalog-​speaking roommate… or maybe I’m too sensitive…
However, I must say, their OT/​PT are very nice, professional and encouraging, which certainly is a plus in the rehab process. Otherwise, again, they certainly can use more staffs.

If I could, I would give this hell hole NO stars. My father was briefly in October 2013.
Gloria, an administrator, took my mother and I around the place like we were on a cruise ship. I had to request the papers to review and sign.
My sister met with the head nurse, who informed her that my father had no medications, but there was a list of at least 10 on Kaiser’s discharge papers. She had not looked farther than the first page!!!
Staff was too busy chatting in the halls to accommodate any requests. The man in the bed next to my father was incapacitated and could not feed himself. Twice my sister, while attending to Dad, witnessed food trays being delivered with no one to help feed him. After minutes, the trays would be whisked away.
My mother and sister were pulled into a meeting with several of the administrators that basically went into finances. One of them suggested they had a friend that would take my sister around to other places for him to live at a cheaper price — since he wasn’t going to get better, instead of discussing compassionate care for him.
After 3 days at this pit, my father was rushed back to Kaiser, his wounds had increased from Stage 3 to Stage 4 and was no longer a candidate for rehabilitation. We had now entered a hospice situation.
I cannot believe that this place is on Kaiser’s recommended list. The place is a nightmare.

I am not sure this is a fair star rating but am up grading it by one star because my previous rating was from several years ago and the facility was under different management at that time. I know nothing about how things are there now.
However, I received a personal message from the current management who at least seems to care what families of patients think. If you have a loved one there I recommend you establish a one-​on-​one relationship with the administration. They can ensure the staff gives your loved one the best care. As for my loved one, I got him the hell outa there a long time ago… long before the current management arrived.

I have to agree with the other reviewers, this place horrible. I, too, experienced the rude behavior of the receptionists, often not even glancing up from texting on their personal cell phones to greet visitors! Needless to say, a continuous stream of people were coming and going without having to sign the register book. My mother had two falls while in their care, requiring an ambulance ride to the hospital for a CT scan and x-​rays, at which time they gave away her bed while we were overnight in the E.R. — which I guess is a blessing. We were not even advised until we were discharged from the hospital that we no longer had a bed at St. Francis. Simple requests, such as a salad at dinner time could not be accommodated, although we submitted a request each time the prior day. Most staff members were friendly and smiled, but any requests for information or care were simply ignored. Although we were assigned 2 days per week for a shower, it never transpired and I had to complain to several staff members about it before that situation was rectified — it was actually a nurse who intervened at that time and made sure that showers were given on schedule. Food and fluid intakes were not measured, noted, or recorded and my Mom lost several pounds while in their care. At the hospital, we learned that she was dehydrated and her sodium level was very low, which may have accounted for the falls due to weakness and dizziness. After one of the falls, a pain medication was administered that should have never been given to her based on the interactions of her other medications and she had a reaction with severe vomiting. Bed alarms were consistently not hooked up, which could also have possibly prevented the falls and when I calmly reported that the falls had occurred, their Social Worker, Dinno, simply mumbled «sorry» and walked away from me in mid-​sentence. Also, their staff gathered up our belongings when we were in the hospital E. R (when they gave our bed away) and put them into «storage», and some of my Mom’s clothing was missing. I was never able to reclaim the stolen property for my Mom. I continuously called Melissa in Admissions and Gloria in Customer Service — to no avail. Calls were not returned and when I was able to speak with them they said that they were still looking in the laundry for the missing clothing and would keep checking.. . Pretty interesting, considering we never used the laundry service there, our family did all of my Mom’s laundry and all of her belongings were in her closet when St. Francis staff packed up her possessions and put them into a garbage bag without allowing us the opportunity to personally pack her belongings. So some St. Francis staff member is now wearing my Mom’s clothing. Beware — do not subject your loved ones to «care» at this horrible facility! Not only is the care substandard, but the staff steals and lies!
Honestly, I could keep going on about the poor quality care at St. Francis Pavillion, but as you can see, this review parallels the others, so need I say more?

My mother is an alzheimer’s resident. The staff is not knowledgeable, and unprofessional. Mostly they prefer to block the alzheimer’s patients behind tables, while the shoot the breeze. They wake my mother up every night to check her blood, then she can’t go back to sleep. When we ask the staff to check her blood before sleeping, they make many excuses about their own interests, not the patients interests. My father visits daily, and I hear many stories about the poor service and poor staff. We are currently looking for a better place.
