C&C Adult Day Health Care is a licensed community based day health care program providing therapeutic health and social services designed to meet the needs of adults and seniors with physical, mental, and social impairment. The Multidisciplinary team of Healthcare Professionals provide a structured day program geared towards optimum rehabilitation and/​or health maintenance of every participant.

History

Established in 1999.

Our company has been responsible for providing outstanding health care services to the community over 10 years. Our participants value our dedicated staff because of their passionate work ethic. And our primary objectives of the program are to: restore or maintain optimal capacity for self-​care to frail elderly persons or adults with disabilities; and delay or prevent inappropriate or personally undesirable institutionalization.
