Practical Care Continuum has been a leader and expert in aging and elder care issues for the Central Texas area since 2000. As the only home care company in the area that utilizes social workers that partner with clients and families to provide solutions for elders, we have formed a solid company on which you can depend.

Meet the Business Owner

Samantha Y.

Business Owner

Samantha S. Young is an innovative entrepreneur with over 20 years of hands on experience, a degree in Health Professions from Texas State and is a licensed Nursing Facility Administrator. After ten years of working in a facility and running a successful non-​profit program, Samantha decided that helping older adults remain independent in their own home and community was her passion in life and with that vision, opened her own home care business.

Samantha has operated, Practical Care Continuum, a successful non-​medical home care agency in Austin since 2000. Samantha is an active member of the American Society on Aging and the National Family Caregivers Association.

As a caregiver herself Samantha understands the challenges and rewards of caring for a parent and juggling personal and professional responsibilities.
