Home Health by Hale Makua is personalized treatment for medical conditions provided in the comfort of your own home, with the goal of preventing re-​hospitalization. Those qualifying for services are functionally or medically homebound, in need of intermittent skilled nursing or therapy services, and have a Hawaii physician overseeing their care.

History

Established in 1968.

Our Home Health by Hale Makua was licensed and Medicare-​certified, with the State and Federal License number 3, in November 1968. We were the third home health care agency in the State, with St. Francis and Straub Hospital being number 1 and 2 respectively. Home Health by Hale Makua has always been Medicare Certified and State Licensed. Since 1997 we have been CHAP accredited by the Community Health Accreditation Program.

The reason for starting the agency was to assist residents to transition safely back home after their discharge from the long-​term care facility. There needed to be safe discharges for patients who came to the skilled nursing facility for short-​term care, and who wanted to return home, but who but still had medical problems or chronic conditions. Accepting clients from the hospital and physicians grew to an overall community need for more care in peoples’ homes after being in an institution, and in some cases, to prevent going to an institution

Kathy D.

Manager

— Kathy Deknis joined the Home Health by Hale Makua team in April 2007 as a Clinical Coordinator and was promoted to Director of the Program in 2009. Kathy is a Registered Nurse and holds a master’s degree in Public Health. She previously worked as an Agency Nurse at Arc of Maui prior to joining the Home Health by Hale Makua team, and has gained extensive experience from positions as a Case Manager, Nurse Trainer, Nurse Supervisor of case managers, Associate Administrator of a home health agency, and Staff Nurse in hospital surgical, neurosurgical and burn units.
