Some of the staff nurses aides I would give 5 stars to the nurses 0 stars, my Dad was brought in the end of Oct 2015 and was taken to Montcalir hospital where he passed on Dec 30 2015 My Dad needed a lot of care and I know it was trouble BUT people your getting PAID you picked your trade… We his children were taking turns going to visit and rub him down with lotion just making sure he was fine but if we were not there it was another story we had to have a meeting with the woman that runs the place twice about my dads lack of care and compassion he would be left with his clothes half off and anyone passing could see that they did not let him have his dignity he was a veteran, a prisoner of war, a paid customer not a freebie. I am not a nurse but one day my dad was confused and withering in pain I asked for a nurse all she did was give him something for pain, COME ON LOOK FOR THE PROBLEM NOT A BAND AID I told her she should assess the situation I demanded she give him a test to see if he had a UTI she said she didn’t think that this is what it was I was not going to leave until they did so she finally relented and the test proved he did have a UTI he was given antibiotics this happened twice. One afternoon I got there and he was soaking wet the whole bed and his covers this happened several times I had to ask them to change him and his bedding .HEY nurses if a patient is confused, looks like their in pain and listless all of sudden wouldn’t you think something is wrong ??? I would ask when was the last time they gave him adavent they did not know it was not documented .I saw people in the hallways unattended screaming for help and staff just passing by, one young man was yelling in his room and one staff member just walked over to his room and shut the door to muffle the sound .My Dad’s nurse was Marybell just hope she never takes care of your loved one I told them I did not want her or another man nurse taking care of my Dad. As far a the rehab staff I can’t say enough about them they were great had my Dad even walking and a nurses aide by the name of Maria took extra care of him. My Dad was taken from there to the hospital with a UTI malnourished, dehydrated and septic shock he was so weak he could not survive this .Oh and I forgot they also said my Dad fell and they found him next to the bed on the floor ???

Not sure where to start after reading the previous reviews. This review is coming from a very protective son’s view. Keep in mind this is not the first rehab facility we have been to.
This has been a great experience for me and my family. My dad is not in good shape and needs a ton of help. They have accommodated him in every way and then some. A couple of people in particular have completely gone out of their way to help us through this tough time (Jason Chipman and Ken Darnell). These two guys are truly amazing at what they do. They have taken time out of their busy schedules to answer any of my questions and or requests. Most of all, they truly care about my dad and our family. I don’t want to leave out the rest of the staff. From the smiling receptionist, care takers, nurses, and PT to Jeff the social worker, these guys are AWESOME!
My dad is coming home this week so hopefully we don’t have to see you all any time soon. But, if something happens we will definitely be back. I cant say thank you enough!

This place is an absolute dump. They have 3 people crammed into a small filthy room. When my grandma was transferred there at midnight, they left her in a wheelchair until 4am.

My aunt is currently a patient, she will be discharged this week. Not sure how I feel yet.

We had a good experience. Rosario, my father’s nurse was fantastic. She genuinely cared about my dad’s recovery and I appreciated that very much. And Jeanette was very helpful and efficient when it was time to leave. A great case manger. Thank you all.

It is horrible that I had to give this facility even one star.
* My uncle is a veteran and this facility claims to be VA contracted. However, they neglected to provide services upon admission and claim to have not known the origin of services regarding my uncle. (Mind you, the hospital faxed all of this information to the facility prior to admission.)
* My uncle was discharged, to the dismay of the administration staff, on December 24, 2015, and I was told that his meds would be ordered through his regular pharmacy. Yesterday, my uncle had been discharged for one week; I called the facility to let them know that he was almost out of meds and I had called the pharmacy only to be told that they had no record of hearing from Upland Rehab. When I called the facility, I was told, «The nurse said she did it.» No one had bothered to call it in.
* My uncle needed his toenails cut when he was admitted. I asked the nurses and assistant administrator to handle it and I was assured that they would. They asked my uncle if it had been done and he said, «Yes.» Mind you, he is s/​p CVA (he had a stroke) and it severely affected his speech. I told the staff this and that he would say ‘yes’ more often than not. They did not even bother to check.
I could go on but I am certain that enough has been noted. I am a Social Services Designee and have worked in the skilled nursing industry for 10 years. I have worked in facilities that have needed some work but this place takes the cake. They are the worst administrative staff I have ever encountered. Their main concern is money and the care of the patient is not paramount in any way. Case in point, I have communicated with the Business Office far more than any other staff member; they sure do want that bill paid but have no idea how to itemize the «services provided.»
I recommend to the other users that if they have the issues they have stated, to call the California Department of Health Services. They can report the facility, as I have, and let the State know exactly what this facility does, and does not, do while they are «caring» for our loved ones. It is awful to think that this is what is available to our loved ones in time of need. What is even more upsetting is that they will get away with it and never acknowledge their faults. I hope that the staff of this facility never feel the way they make residents, patients, and family feel.

Worst experience ever. Filthy rooms. Bathroom that smelled like pee. Forgetful staff. My mother was there for 3 days and is a diabetic. They had no diabetic diet for her and every single time they brought food it was something she was unable to eat. When they said they would bring her something else they NEVER CAME BACK. The nurses took an hour to change the lady’s diaper in the bed next to my moms on several occasions so the room smelled like feces. They left a commode full of urine in the room for the entire 3 day stay. The nurses are nice but nice doesnt cut it all the time. The call button for my moms bed was not plugged into the wall so my mom was calling and no one came AND the staff was aware of it because when we finally got someones attention they knew the button was not working and they mentioned how they gave it to the patient in the bed next to hers. Then proceeded to give her the working call button and leave the other patient with no call button at all!!!
Horrible horrible facility. It was HELL.

Negative a trillion times over and over again, and it’s suppose to be one of the better care facilities. The experience my precious Mother had was the worst care facility no love, the staff is all a façade the activities don’t exist just on paper. My mother was left in diapers for hours, before she went there she was not in diapers she was able to use bathroom with assistance !!! I had to always ask questions but everyone would disappear and leave it up to the nurses station. I only encountered a woman genuine enough to do her job thank you Leeane Padilla you don’t belong working there with staff members who are heartless!!! From the administration they never return phone calls they hassled me about releasing my mother!!! My mother and my family had every right to get her out!!! Her rights were violated on every level her dignity was taken away!!! Medicare needs to know how you treat the elderly they deserve better care. If you have a loved one don’t place them here !!!

Alright. My mother is 84. I was incredibly concerned as to where she was going to go for rehabilitation following a major surgery. I decided to actively check out the rehabilitation centers in the area to ensure I had the right fit.
Right off the bat I was very worried about most of the ratings and, especially, the comments here on Unilocal of ALL the facilities in the area. I relaxed somewhat when I figured most of the ratings were from people with expectations that were either too high or had some type of a run-​in with staff and were trying to get their revenge.
Admittedly, I walked into Upland Rehab and, at first sight, didn’t like how old the facility was. It even had an «old smelling odor» emanating throughout the interior. But I certainly couldn’t complain. The facility was otherwise clean and filled with the hustle and bustle of staff ranging from Doctors, Physical Therapists, Nurses, LVN’s, CNA’s as well as Administrators. Everyone is easily identified with their identification card hanging from their clothing or uniform.
I met with one of the Administrators who was friendly and gave me a tour of the entire facility. As we walked I was introduced to staff members who not only said hello but asked about my mother and spoke to me about their function in the facility. I spoke to the Physical Therapist who told me what her plan would be should my mother come to their facility. I liked the way the facility was set up and how each wing of the facility served as a different type of need for their patients with the appropriate staff. Staff-​wise, there was plenty of STAFF to PATIENT ratio.
The welcoming for my mother as she was wheeled in by gurney was warm. Staff members along the corridors all said hi and made her feel welcomed.
My mother was a patient of this facility for almost two weeks. During that time there was one «hiccup» which I was not pleased with but, in whole, her entire stay was very positive. Staff members were incredibly polite, professional and, just as important, very caring a nurturing. I was very pleased with each staff member I came in contact with. Myself as well as other family members were there multiple times each day and night. Some nights I stayed until late in the night visiting. Although there are official visiting hours, family members can visit anytime day or night. Even in the nighttime hours one will find tons of staff in the facility busy moving about assisting their patients.
On the day we left, once again, staff members came to her to wish her a speedy recovery and some even hugged her telling her she would be missed. In conclusion, I see what the other folks are saying about Upland Rehab with their low ratings… but my experience, and my mother agrees, was terrific. I feel very fortunate to have had this facility so close to home (6 minutes) and to have had my mom treated by these fine folks!

Negative a million stars if possible!!! AVOID AT ALL COSTS.
My grandmother was here last month, and the service was terrible. When my grandmother needed assistance using the restroom, the nurse and other staff made her wait more than 20 minutes!!! No one was willing to help. My mother and I did it.
My grandma passed away there also. My aunts were there the day before she passed and they said the nurse Roxana gave her 4 pain pills when my grandma was saying she was uncomfortable. The next morning at 6:45AM she passed away. Not sure what kind of big white pills they were.
The whole morning and a few days before my grandma’s passing, I was witness to the complete negligence of the lady in the bed next to my grandmother’s. She was considered to be kind of wild that needed constant supervision. Despite that being known, the lady was in the bathroom all alone, creating a flood, and she was also grabbing all sorts of clothes from her closet and putting them into the bathroom trash and filling it with water. She was trying to wash her things. Also, she would wander the halls and no one could keep track of her.
I had to alert a random nurse that the lady in 23A was making a flood. And she said «Oh no why is she along?» In my head I thought, «She’s been alone for more than an hour.»
This place is an absolute nightmare and they should be shut down.

My 71 year old father had a total knee replacement surgery and was uncomfortable going home after 4 days as directed by his dr. He lives alone and has a 2 story home so we felt a rehabilitation facility would be beneficial to his recovery.
He had strict instructions from his dr’s and physical therapists to stay in front of his pain and to make sure to take pain meds on time or extreme pain can hinder his recovery. When we arrived and he was admitted by the nurses, he asked for the medication he was overdue to receive. He asked several times and was not given ANY meds until 5 hrs later. I was concerned but hopeful this was just a fluke due to him just arriving.
The next morning he was given a medicine cup with his «morning meds». He asked what they were and the nurses response was «your morning meds». He asked again what particular medication each pill was and she named them which included aspirin. He told her he was extremely allergic to aspirin and she responded — «this is what the dr ordered». At this point I was extremely concerned for my fathers safety and insisted to speak to someone in charge.
My father was there for rehabilitation and did not start any therapy until day 2. Once his therapy began, the therapy staff — Twinkle and Ken were fantastic but the management at this facility was absolutely incompetent!
Keep your loved ones away from this facility!

There are a lot of caring and compassionate people who work in this nursing home. I have been to many nursing homes and you can really tell that the employees here are working hard to take good care of our loved ones.

My entire family was VERY UNhappy with this «Rehabilitation & Care Center.» They run like one of the poorest convalescent homes you could imagine. There are people (including our elderly family member) that are unable to get out of bed alone or see to know where the water cup is. We all timed how long her call button was on before someone came and how many CNA’s walked by the light without stopping. She had a clip attached to her gown that would set off an «alarm» if she tried getting out of bed… you hear these alarms ALL the time with NO ONE rushing to make sure the person is OK. They do not have signs that notify staff when someone is hearing/​vision impaired. I had to make our own but it didn’t do any good. People would come in and out, speak at a level that was hard for ME to hear and not announce who they are — in turn frightening my grandmother. I was there every single day for almost a month and witnessed the CRAP they like to call care. When I would finally go into the hall to track someone down to assist my grandmother with getting to the bathroom, I was actually blown off and ignored. I’ve witnessed conversations in the hall between staff regarding patients and not regarding their medical care. more like talking crap and complaining about what they have to do. One nurse told me «AGAIN?! I’ve already changed her» when I told her my grandmothers «diaper» needed to be changed (She was diapered because they were not coming in time for her to get to the bathroom to use it.) I could go on and on about our experience. My grandma did nothing but decline while she was in this facility. I am VERY glad to have her out. Administration is full of it when they say they have changed things. Remodeling the acual facility does not mean that the staff have been trained and REtrained on how to PROPERLY care for these patients.

After viewing their promotional videos of their facility online;
We went to this facility for a brief tour a week or so ago. And compared to what they say in their videos, things were not as they seem.
They say they compare their services like the Ritz Carlton (or some upscale hotel). What I saw just when I walked in was more confusion and disorganization. Their videos say they respond to their patients after the first call light. Just waiting in the main lobby, the call lights were constantly beeping non-​stop. Patients were wandering through the hallways, lobby, everywhere. We met with the admissions rep and she spoke so fast, just after walking down one hallway of the tour it was as if she was talking us out of considering their facility. We just got a bad, uncomfortable vibe there just int he first 5 – 15 minutes of looking around.
Also, the admissions person discussed our situation openly in the hallway with patients all over the place — with no professional courtesy of privacy. Perhaps we could have talked in her office or work area first, then follow with the tour.
The whole experience was just uncomfortable, NOT like the Ritz Carlton at all. Call lights would not stop beeping. One would have to wonder how consistent their care and promises are when they’re promoting one thing and yet their facility practices are completely something else.
