Take Care Advisor’s registered nurse geriatric care managers help older adults achieve better management and control of their total health care, longer independence, and greater peace of mind for themselves and their loved ones.

RN Geriatric Care Managers do not provide clinical care, but rather they manage and navigate the client’s health care system and ancillary services, from doctor’s appointments to their private duty home health care services.

Our services are designed to meet the individual’s needs and goals and our services are adaptable to their changing health. Take Care Advisor’s programs are flexible and require no long-​term commitment.

Some of those services include:

*Assisting Clients in crisis and other times of need

*Conducting an initial assessment that provides a perspective of their relative health status

* Visiting and calling Clients to establish regular contact, which may include coordination of upcoming doctor office or clinic appointments

History

Established in 2008.

Susanne opened Take Care Private Duty Home Health Care in Sarasota, Florida fifteen years. Since then she has opened offices in Bradenton and Venice. Furthering her vision and mission to bring her clients the highest quality in care, she intuitively forecasted the need for geriatric care management services, opening Take Care Advisor in 2008.

As owner and entrepreneur of Take Care, Susanne pays special attention to two crucial elements: quality of personnel and customer service. The personnel are the foundation of how Take Care is able to provide clients with the highest level of geriatric care management services, private duty home health care, and non-​emergency medical transport services. With the goal being to serve clients to the best of our ability, our employees reflect this commitment to Take Care’s standards of excellence.

As Executive Director for Take Care Advisor in Sarasota, Florida, Courtney coordinates and facilitates daily office operations among the Take Care Advisor’s Geriatric Care Managers and their clients.

Courtney is an active member of the Greater Sarasota Chamber of Commerce and the Young Professional Group in addition to volunteering for Big Brothers|Big Sisters. Courtney was selected as a «40 Under 40 winner by the Gulf Coast Business Review. In addition, Courtney was chosen as a Young Professional of the Year finalist in May 2010. Biz941 selected Courtney as «People to Watch,» a yearly program that highlights rising business people and movers and shakers who are doing something new and innovative in the business world. Courtney is helping to propel Take Care Advisor to the forefront of health care integration.

Courtney received a Master of Science in Gerontology degree from the University of Southern California’s Davis School of Gerontology in Los Angeles, California.
