History

Established in 2004.

Dr. Lewis had the opportunity to attend and receive my Undergraduate Degree at the University of Utah continuing on to achieve his Doctorate of Dental Surgery (DDS) at the Creighton University Dental School in Omaha, Nebraska. After schooling Dr. Lewis opened his practice in Sandy, Utah.
