Nursing & Rehab At Home is a clinician-owned, Medicare Certified, licensed home health agency serving the San Francisco Bay Area for over 10 years. Services provided include in-home skilled nursing, rehab, home health aide and nutrition services. Individualized care plans are developed in coordination with physician, patient and families. Our goal is to improve the patient’s quality of life so they can reach the highest level of independence and safely remain at home.
History
Established in 2001.
Lorna Beukema, Physical Therapist and Owner, started the company in September of 2001 after Stanford University Hospitals & Clinics closed their Home Health Department earlier that year. Lorna, who had worked at Stanford for 18 years — mostly in management, hired 7 of her recently laid off colleagues (nurses and therapists) and started Rehab At Home, Inc. The company became licensed in December 2001 and became Medicare Certified in March of 2002. In 2011, the company changed it’s name to Nursing & Rehab At Home to reflect it’s comprehensive service offerings. Currently, the company has 50+employees and has cared for well over 10,000 patients.

Business Owner
Lorna Beukema is a Physical Therapist with over 30 years of experience in a variety of capacities including direct patient care, management, and administration. After working several years in Acute, Rehab and Skilled Nursing facilities, Lorna was employed in the Home Health Department of Stanford University Hospital & Clinics from 1985 until it closed in 2001. Following the closure of Stanford Home Health, Lorna started Nursing & Rehab At Home, (formerly Rehab At Home, Inc) a licensed and Medicare certified home health agency in the San Francisco Bay Area. In 2011, Lorna founded Agility Health, a licensed private duty home care agency. She is the sole owner and CEO of both companies. Lorna also currently serves as Treasurer of the Bay Area Directors Forum, and is an active participant in APTA, CAHSAH, and NAHC.
