We offer in-​tensive stroke and orthopedic rehabilitation and 24-​hour nursing services. We have achieved impressive outcomes as a result of our state-​of-​the-​art programs, equipment and nursing services.

At Plum Tree Care Center, we believe that each person in our care in unique, with individual needs and requirements. This philosophy directs our Total Care approach to patient treatment. Total Care begins with an intense focus on the medical needs of each of our residents, then continues with attention to their personal, social and spiritual needs as well. My loved one had a major stroke, and after a month in the hospital and the acute rehab center, I chose Plum Tree Care Center after it was recommended to me by a nurse. My partner spent 2 ½ months there and I can’t say enough about how caring the entire staff was to both of us. We were treated with such a level of compassion I had never experienced. There wasn’t an issue that couldn’t be solved. The dietician and the social worker even went shopping for foods that he would eat! We, also, enjoyed a beautifully prepared Thanksgiving dinner with all of the residents and their guest. The meticulous attention to making sure that he had a compatible roommate was really appreciated.
Since I was there almost every day, the center became a second home to me. I felt confident when I had to go to work, that he would be given the highest quality of care by each and every staff member at the center. The rehab therapists and nurses were highly committed to his recovery. He came home last week and continues to recover with physical and occupational therapies. We will always have fond memories of our experience there and will continue to visit the residents and staff who we grew to know and love.

My family and I cannot say enough about the Plum Tree Care Center. Although my grandma was only a resident for a day before she passed, we were thoroughly impressed by their compassion and professionalism for her and our family. Their facility is clean, bright, and cheerful. The nurses and other staff were welcoming from the start, treating my grandma with dignity and respect, while still providing us with the time we needed to be with her in her last moments. We would not hesitate to recommend this facility to anyone looking for themselves or a family member.

My dad was at Plum Tree for several weeks and he had outstanding care. The facility is very clean and most importantly the staff is attentive as well as caring. The physical therapy my dad received was extremely focused and beneficial. He came home in better condition than before his fall! Thank you to the Plum Tree team!

Mom needed a place to stay after being in the hospital. We picked Plum Tree because it was clean, bright and had a good recommendation. Mom is having a positive experience and gettting better. The staff is wonderful and always very helpful. There are lots of activities going on including bingo, movies and many more. When I leave Plum Tree after visiting with Mom I feel very comfortable knowing that she is in good hands. Plum Tree is a blessing!

My Father has type 1 diabetes. His bones are getting weak due to this terrible disease. He broke his right ankle area in two places and needless to say was having a very difficult time with thoughts in the back of his mind of an amputation… physically and emotionally… he was sad. After several weeks in the hospital he was admitted into the Plum Tree Care Center. First visit for my dad to a care center so we had no idea what to expect. When we admitted my dad; my brother, mom and I were greeted by the staff management and were happy with what they had to offer. A warm and informative admittance left my family and my dad feeling good about his rehab future.
My father had one of the best experiences with the staff, patients and most of all his rehab. Believe it or not they even helped us throw a surprise 57 year Anniversary Party for Mom and Dad, with a piano performance, a beautifully decorated hall and an outstanding catered lunch, so happy my brothers, sister and I provided lunch for the entire center…
Being sick or injured is never a fun thing but my dad felt so cared for… we have recently visited the center and had dinner with the staff and patients. We helped another family celebrate their parents anniversary. It was nice to be there as invited guests… My father visited with the staff and friends who were still patients there since he had left. They shared stories, a meal, hugs and then… sad good-​byes. We will be back there I am sure, for a visit or future care for my folks if ever needed… :)

We never even got my Mom into this place. First contact they were great, positive and ‘of course’ they would take Mom. Later the same day they started lying to us with different excuses of why they couldn’t take her. They couldn’t even keep their lies consistent. Next day; more lies. Contacts were with MaryBell and Mira in admissions. They, again, said they would take her but when we went to check her in — they came back with ‘no’ saying she had too much history with medical issues to take her. They didn’t even have the ethics to be honest with us from the get go and we spent hours getting her they just to have her refused. Don’t touch this place. If they were lying to us even before our mother was admitted — what else do they lie about.

My mom has been at Plum Tree for the last 5 weeks. My mother had hip replacement surgery and came to Plum Tree for healing and rehab. The Staff has been GREAT! The nurses are warm and friendly and the OT and PT Team is very, very good. The facility is clean and warm and comfortable. I looked at several places before deciding on Plum Tree. I picked Plum Tree because it was the most well kept, light and cheerful place and they had a great rehab team. As for the nurses and aids, it’s difficult to make an assessment until you are actually staying there. I have to say that I am EXTREMELY pleased! The admissions staff members are very nice and well organized. They have good food for the residents, and have entertainment and activities as well. It’s a great place for a loved one for either a short or long stay.

I stayed at The Plum Tree Care Center while recuperating from a hip fracture. I found the staff at Plum Tree Care Center to be caring and supportive. Everyone, from the nurses, CNN’s, therapists, assistants, were all extremely helpful and attentive. They bent over backwards to make sure that my staff was comfortable and my needs were met. They were with me every step of the way in helping me progress through my injury The physical and occupational therapists were wonderful — patient and caring .The management was also very concerned that my stay was made as comfortable as possible. If I had a problem they would do I they could to resolve it. They were personable and supportive. When I was upset because I was not cleared to walk on my own, they went to great lengths to explain to me that it was because they wanted to make sure I was not in danger and that it was a safety issue. When I could not get a connection to my laptop, they responded right away and assisted me with getting a connection to the internet. When I turned my call light on, the response was almost immediate. The food was also very good. I was surprised that the quality of the food was as good as it was, but I enjoyed the food very much, with the exception of the coffee, which could stand improvement. The facility itself was clean and well kept and there is a very nice courtyard and dining room area. I would highly recommend the Plum Tree Care Center to anyone who is looking for a caring, supportive, clean place for they or their family member.

The building is old. It is what it is. My mother in law is in and had an unwitnessed fall. After she screamed for help a group of nurses came in put her in a chair at the end of the bed and then the whole group left together with out a call light with in reach, She had to scream again for help. Soooo lets just say I am getting her out of there as soon as I can.

Everyone has a right to their opinion, hence several negative reviews. Plum Tree is one of the more caring and clean convalescent homes in the area. The staff are wonderful, caring, and respectful of their patients and the patients family. Please don’t judge this fine facility by a few negative opinions. It’s not perfect but neither are many of the others.

My mother was under the care with Plum Tree for about 2 months and it was a really bad experience from the lacking of caring about the patients to the very slow respond time from the CNA once a the service bell was rung at the bed side (sometimes up to 15 min wait). I had to hire a Care giver to come to the Plum Tree four days a week because I was concern about the lack if care and outright neglect of my Mother. The head of rehabilitation Carol White was rude, condescending, and aggressive and was threatening numerous times when we were discussing my Mother’s care. Plum Tree only cares about the bottom line and how much money they can bill Medicare but there is no real caring for the patients. My Mother has a good 2nd level insurance as well and I felt that they wanted her out of the facilities soon after the 21 days since Plum Tree makes the most money from Medicare in the first 21 days… Know your rights if you send a love one to the Plum Tree.

They were fine at first in taking care of my grandmother. Then they forgot to call for transportation to a doctor visit, then forgot her paperwork to take with her for a doctor visit. She has been on oxygen for at least 6 months, and within the last 3 days, they’ve only had her on oxygen per our request, even her chart saying she needs to be on oxygen. Staff may be overworked, but making sure a patient is on oxygen when it can be life or death isn’t too big a thing to ask.
