History

Established in 1995.

Cascade Eye & Skin Centers, P.C., is a large group of board certified physicians specializing in ophthalmology, laser vision correction (LASIK), and dermatology, and it is one of the most successful medical practices in the western United States.

In 1995, Cascade developed as a union of two leading practices in western Washington — Tacoma Eye Clinic and Eye & Skin Associates. The original Tacoma Eye Clinic had roots dating back to 1967.

Today Cascade employs over 30 eye and skin providers (physicians and physician assistants).

Cascade Eye & Skin Centers, P.C., continues to grow as more residents of the south Puget Sound area recognize the value of their unique style of high-​tech treatment and personal attention to the needs of each patient.

Business Owner

Dr. Gildenhar grew up in Indiana. He loved roaming the hardwood forests and beautiful farms, swimming and fishing in old stone quarries, playing basketball and golf, and being out with his friends and family on warm summer evenings. He attended college at Northwestern University in Chicago, where he met his needs for fresh air and space by playing on the golf team, taking long jogs by Lake Michigan, and spending a few spring afternoons in the bleachers at Wrigley field.

He attended medical school at the University of Cincinnati. It was there that his love for ophthalmology began. After three years of ophthalmology training and sampling big city life at Northwestern in Chicago, he moved to Tacoma, Washington, where he has been in private practice for 23 years.

So appreciate Dr. RAUSCH! Great doctor who explains things very well. He will answer any questions and not make his patients feel rushed. The only reason I didn’t give five stars is the «check in» people are a little «hurried». A warm smile goes a long way to making clients feel welcome.

I would not choose this clinic. I have been to this clinic before and was more impressed with the optical department, not the providers. I waited two months for this appointment. Today I had an appointment but a half hour later I was still waiting. I asked how much longer and was told I was next. I had another appointment today so I wanted to be out in one hour. I was told they could not guarantee that since it takes a while for eye dilation. I felt they could have made an effort to finish my appointment in one hour since they were the ones running behind. I don’t mind waiting if I know I will be given great care. I felt they could care less. I won’t return and I do not recommend this place.

I’ve been going to Cascade for several years now. I first had services in Puyallup until the Bonney Lake office opened. My experience has been decent overall, and sometimes hit and miss. It is a little odd for the tech to be doing almost the whole vision exam, except the dilation. I don’t know if this is just a Cascade thing since they are such a large office. I first switched to Cascade because my former incompetent optometrist did not recognize an inflammation I had due to protein buildup on my contacts. That first visit was good and bad. The tech was AWFUL. She was condescending and almost berated me about not cleaning my contact lens cases (by that point I’d been wearing contacts over 20 years). However, the opthamologist was awesome. I believe it was Dr. Harris. Then I started seeing Dr. Croft. She is nice, but there were a couple of years she just couldn’t get my prescription tuned in (I am severely nearsighted). I’ve never had any other problems with any other techs.
Once I switched to the Bonney Lake office, the results have been hit or miss. Most of it is due to the rotating opthamologists, so it seems you’re never guaranteed to get the same person. Dr. Drouillard had wonderful bedside manners. Last year I saw Dr. Shields. He was nice enough but abrupt. He advised me of a procedure for my extreme nearsightedness, which I appreciated. However, he was abrupt in that he repeatedly asked, «Any more questions, any more questions?» when I hadn’t really had time to think of what I might need to ask.

I didn’t even schedule an appointment because I was so disappointed in the behavior of the appointment scheduler. She doesn’t seem to know what insurance covers what and when I told her my insurance covers routine eye exams, she said «ok, but just so you know, if there is something medically wrong, you are not covered and will have to pay for it.» Based off the other Unilocal reviews, it sounds like this place is one to avoid anyways.

Called for an appointment because my contact lens prescription was expired. Arrived for my appointment and waited and waited clearly my time wasn’t important. Was told we can’t fit you today but can get some trial lenses. Docs at Costco can figure it out maybe they could help these guys so not only am I still waiting to see the doc after over an hour but I get the pleasure of coming back to wait again for their disorganized assembly line approach to patient care for a contact fitting. Seems like a machine does that for them maybe if they had the instructions handy we could not waste anymore of my time and could get it figured out if it’s the green or blue button to press. My suggestion would be to go anywhere but here for anything you need eye related unless you are excited to hang out and wait. I’m guessing my complaining has earned me a little extra wait time just so I can be completely clear on what a disorganized mess they have going getting close to the hour and a half time finally got 3 minutes with the doc get to come back for a fitting wonder if I will have to take some vacation time.

Unilocal needs to allow negative stars because that what Cascade deserves. In particular Dr Fox. The experience I had there a few years ago would read just like any horror story you might see in rag mag and it haunts me to this day.
I made an appointment there because I was having some trouble reading small print, glasses, right? Nope, after the exam I was told I had cataracts in both eyes and they had to come out. Surprised, I always thought that was an issue for the elderly.
The procedure on the left eye went well enough, in and out. then the right eye and the beginning of a nightmare. During the procedure I felt Dr Fox applying allot of pressure to my eye then I felt, and heard a POP. large black tendrils of blood quickly filled my eye, I remember that they looked like cobwebs… it wasn’t but a few seconds and other then a few very small slivers of lite, I was blind.
Panic set in, I wanted to bolt from the table and run out the door but I knew he had to finish this grizzly task. Once completed Dr Fox calmly told me that the black tendrils would clear up in a few days and sent me on my way. Over the next few days it became clear to me that there wasn’t going to be any clearing up in my vision, in fact it got worse, then the swelling and ocular hypertension set in. Dr Fox called me over the weekend to see how i was doing, once informed he had me come in where I was then sent to another retinal surgeon who said I now had to have another surgery (vetrectomy) to fix all the screw-​ups from the first one (cuts, partially detached retina, vitreous hemorrhage, and more). All of this cost my insurance company tens of thousands of dollars and left me holding the bag on thousands as well.
To this day I have blurred, fuzzy vision highlighted by giant halo’s at every light source and beings that I make my living from reading plans and documents it goes without question that this has drastically impacted my life
I could go on and on about this dark episode but I believe I’ve delivered a fair warning to the people.

Last year I somehow got a piece of metal stuck in my left eye. I went to the hospital, but it was so deep they wouldn’t remove it because they were not equipped to do so. They let me know I would have to go to an eye surgeon and get it dremeled out. They referred me to Cascade and the nurse left them a message that I needed to get in asap because I was in excruciating pain and was risking severe infection. Cascade called me the next morning and said they couldn’t get me in until the following afternoon. I let them know I was in a ton of pain, all the vessels in my eye were broken, and that I hadn’t slept in 24 hours because every time I would blink it would make my eye gush tears and gooey stuff. The response of the woman was, «Normally we schedule people several weeks out because we are so busy, so you are lucky we are even fitting you into our schedule at all!» I would of told the woman to «f*** off», but you need to be referred to an eye surgeon, and I didn’t have a lot of options for getting into one immediately.
Anyway, when I went there I had to wait an hour past my appointment time to be seen. The doctor looked at my eye, for less than 5 seconds, and claimed there was nothing in it. My entire eye was blood red and constantly dumping tears, but he claimed nothing was wrong. Finally, after begging him to look again he did… and guess what?!? He found there was a giant piece of metal in my eye. His response, «That is too big and deep to take out without permanent eye damage. You should of came in right after it happened, because your eye has already started healing over it which is why it is too deep to remove it now. Maybe call earlier next time.» I started to explain how the clinic wouldn’t let me come earlier despite the hospital claimed it was an emergency, and he just walked out.

Went here for a couple years, only for fillers and Botox… Had an appointment I needed to reschedule so I called and left a VM (it was after hours) letting them know… Called back during business hours to make sure they received it and the lady I spoke to was totally nice and understanding advised me to call back when I’m ready to re-​schedule. Called back a week or so and talked to another older sounding lady she advised that I owed $ 75.00 for canceling before rescheduling anything was extremely rude and said because I left it on the main lines VM and not the cosmetology departments VM I owed the cancelation fee. I always called the main line to et to the cosmetology department did not know they even had a separate line. I stopped going there because the lady was soo rude, mind you I have spent $ 1000’s of dollars in services there. I now go to federal way laser skin– my first visit there I was told my filler was being in accurately placed causing it to not last as long — this is a fact, ever since switching to federal way laser I’ve notice a difference in the time in between visits and all over look. Dr. Sabboth specializes in face structure and doing what is right for you. In the end I’m not happy with the treatment cascade eye and skin care displayed but happy it happened because it lead me to a place I am much happier with.

Had to give one star but really they deserve none. I went here because my insurance changed. I had been going to Dr. Liljas and loved it there. This place is huge and impersonal. I was given a light up pager which was ridiculous. Do I get a drink and appetizers with my eye exam? I then was taken back by one woman. Then another woman came in and started my exam. I consider myself very good with accents but this woman’s accent was so heavy and she spoke so soft I had to ask her to repeat herself 10 or so times. I then was moved to another room for further testing then moved AGAIN to the exam room. The dr. came in and I was moved again to another room for an additional test then back to the exam room. The whole thing was a joke. I saw three people and was in 4 different rooms. The dr prescribed some lenses that I didn’t like so I had to go back. This time I went to the Bonney lake office which was MUCH better. I will not go to the south hill location again. Too big box for me.

Speaking in regards to the eye department. The staff are friendly, but… The wait times can be a while past your appointment time. It doesn’t feel so cozy, either. I never know who is dealing with me. I often end up seeing a bunch of different people, and I don’t know who they are and why they keep switching… When I finally see the doctor, it doesn’t seem the previous nurses relay any information to him so I end up always repeating myself. They ask me the same questions over and over, each visit, and don’t seem to care about achieving happiness with my eye sight as I struggle with a lot of astigmatism. They frequently throw me from room to room and I go through the appointment not knowing what’s going on. On the upside… They are good about appointment setting and times, so it isn’t difficult to get in. I did get in on a last minute notice when I had something lodged inside my eyelid, and the doctor was able to get it out efficiently.

Not good. I went in because I have been having trouble seeing things at far distances, especially in my college classroom. After going through the eye exam and having my eyes dilated, I was seen by the doctor (for 2 minutes max). He must not have communicated with the nurse who started the exam, because he didn’t have any of the info I had given her during the first 20 minutes or so. Eventually I was told that I was young and my eyes were fine, maybe sit a little closer to the board in class. I left the clinic thinking «but I still can’t see!», so I went to another eye doctor, who looked deeper and wrote me a prescription for glasses which have really helped. Skip the trouble of going through an eye exam twice and avoid this place!

AWESOME experience at Cascade Eye and Skin! Everyone was extremely friendly and helpful from the front desk personnel to Dr. Brady. I will continue to see him for all of my eye health care. I tend to have several questions and he was quite patient with me and answered all of them. The techs were great too!

This review is strictly regarding the cosmetic dermatology area, not the dermatologist. I’ve struggled with rosacea for years and decided to give the IPL treatment a shot. After 3 or so treatments, not a huge difference. On the fourth, one side of my face was so severely burned, and now scarred, I’ve tried several things since and am still pursuing options, but not here. I missed about 2 weeks of work for the boils to scab off. Not a pleasant experience and would not wish it on anyone. Essentially it took a problem someone would already be self-​concious about and made the problem worse.
