Refuse to provide service because I didn’t want to sign the HIPAA form. I hwas asked by three people (the front office staff, the dental assistant and the office manager). Each time, I informed them that I choose not to sign the form and they could mark me down as «Individual refuse to sign». I was informed by the office manager that I could not be seen.

My best dental experience ever!
I was very nervous at first, but the staff was so welcoming and made me feel so comfortable.
Dr. Moss explained to me using pictures the treatment I need, and Amanda explained everything to me again.
The office is very modern, and clean.
I would recommend Smile Forever Dentistry to all my friends!
