ChapCare has been putting patients first since 1998. For 17 years ChapCare has served the communities of the San Gabriel Valley, providing quality health care for men, women and children of all ages.

Who We Serve

ChapCare serves all residents of the San Gabriel Valley. We contract with a number of qualified health plans to provide primary healthcare services for those with health insurance. ChapCare also serves as the community safety net by providing comprehensive health care to low income and uninsured individuals and families.
