Legacy Home Care, is leaving a legacy of caring from the heart throughout Maricopa County since 2007, by providing non-​medical home care and geriatric care management to seniors and chronically disabled who want to remain in the familiar surroundings of their own homes.

Our mission is to exceed our client’s expectations by providing quality service to help our clients lead dignified, independent lifestyles in the comfort and safety of their own homes. We carefully assess the client’s needs and selectively place screened, bonded, insured, trained personnel to meet those needs.

** Non-​Medical Home Care Services

Our highly qualified and trained caregivers are available to assist with:

* Personal care * Light housekeeping

* Bathing & dressing * Companionship

* Errands * Medication reminders

* Meal preparation * Respite care

Caregivers are available ²⁴⁄₇.

** Geriatric Care Management & Transitional Care Coaching

* Conduct an assessment and develop a care plan for individuals and their families.

* Screen, arrange and monitor in-​home medical and non-​medical services.

* Accompany clients to doctor appointments, as needed.

* Chronic disease education and coaching: AMI, CHF, COPD, Pneumonia, Asthma, Diabetes, Fall Prevention

** Assisted Living Placement

For those unable to stay home, we assist in the daunting task of finding, touring and evaluating the best assisted living option for your loved one based on budget, location, and care needs.

Meet the Business Owner

Jamie F.

Business Owner

Jamie Flesher is a Registered Nurse and a Certified Geriatric Care Manager with over 29 years of experience in the health care field. Her work history has afforded her the opportunity to work directly with the elder population and individuals with disabilities. This has provided her an understanding of the enormous challenges faced by individuals during the aging process especially those experiencing age-​related illnesses.

Jamie has been the CEO of Legacy Home Care, LLC since 2006 and has achieved the Care Manager Certified (CMC) designation through the National Association of Certified Care Managers. She is a member of the National Association of Professional Geriatric Care Managers, a member of the Arizona Non-​Medical Home Care Association, a member and State Coordinator for Elder Care Matters Alliance, and serves as a Co-​Chair for the East Valley Committee on Aging.
