John W. Jaureguito, M.D. has been treating patients in Fremont and the Bay Area since 1995. Dr. Jaureguito graduated Cum Laude from the University of Notre Dame, and was an Alpha Omega Alpha graduate of Saint Louis University School of Medicine. He completed his residency at the University of Chicago and his fellowship training in sports medicine and arthroscopy at the Orthopaedic Specialty Hospital in Salt Lake City.

Dr «J» is passionate about treating physically active people of all ages from high school and college to recreational athletes. He believes in a custom tailored approach and creates a personalized plan for each and every patient.

Dr. Jaureguito is the Medical Director of FORM and the FORM Sports Medicine Program which specializes in cutting edge treatments for knee, hip and shoulder conditions. He has treated tens of thousands of knee, hip and shoulder patients.

John W. Jaureguito, MD is the Founder and Medical Director of FORM as well as the Medical Director of the Fremont Joint Restoration Center. Dr. Jaureguito founded the FORM Sports Medicine Program, a state-​of-​the-​art orthopaedic surgery practice dedicated to excellence in the treatment of sports injuries.

Dr. Jaureguito treats all sports related injuries and is an expert in arthroscopic and reconstructive surgery of the knee, hip and shoulder. He is fellowship trained in sports medicine and arthroscopy, having spent an extra year of training and research at The Orthopaedic Specialty Hospital in Salt Lake City.

Dr. Jaureguito cares for numerous high school, college, professional and recreational athletes. He is Team Physician for Cal-​State East Bay and Ohlone College. In addition, Dr Jaureguito is the Team Physician for local high schools Mission San Jose, Newark Memorial, Moreau and Milpitas High School.

My teenage daughter was injured from a fall and required 2 surgeries by Dr. J. He and his staff were very personable and extremely professional. We did not wait long and he really explained the procedures well. The office and surgery center were very well organized and clean. Most importantly, my daughter has done extremely well. She has healed nicely and is on the road to recovery and return to her sport. Thanks Dr J and staff!

This is a long-​overdue follow-​up review. Unilocal has put my first review in the ‘not recommended’ area. Why? A mystery.
After many years of pain in my right knee Dr. Jaureguito did a partial knee replacement for me in September of 2013.

I love to go hiking but the knee had become so tender and painful that I could only walk in flat areas and ONLY on even (paved) surfaces. Any twist or turn of the ankle would result in knee swelling and pain that could last anywhere from hours to days. Icing did not always bring the swelling down. Within hours of my surgery I could feel the difference. With the physical therapy afterwards the muscle strength returned quickly. I started walking again, a little bit further each day. With no more pain from walking on gravel and scrambling over rocks I got brave. I applied to the permit lottery for hiking Half Dome. Amazingly I won spots for me and several friends. Our assigned date was in October 2014 — just a little over a year after my surgery. My husband and I started training hikes nearby to be ready for the 17-​mile round-​trip that is the Half Dome hike. We started with distances of a couple of miles and then graduated to the steep Rocky Ridge trail at Del Valle. Next we conquered Mission Peak up the back side from Sunol which is 10 miles with some steep and rocky areas. The first time up Mission Peak it took me almost 8 hours to complete. As I grew stronger our time shortened to a tad under 5 hours. By October I was ready! We started from the valley at 6:00 am and finished at 7:00 pm. WOW! I am grateful and owe much to Dr. Jaureguito for his care and his skill. He is an outstanding surgeon and his support staff is right up there in the outstanding range with him. Many thanks.
Post Script — first hike of Half Dome at age 65! YEAH… if I can do it (with Dr. J’s help) then you can, too!

I had arthroscopic knee surgery done by Dr. Jaureguito in 2014 for a torn meniscus. My rear ended up being reparable so instead of shaving off part of the meniscus, Dr. Jaureguito repaired it for me. Although the rehab time is considerably longer for repairing the meniscus vs. shaving it, it is also better for you in the long term.
It’s 2015 now and my knee feels like it was never injured. No pain at all, even after strenuous physical activity. Whether it’s running at the gym or playing sports, there’s no pain.

I had bilateral partial knee replacement by Dr. J in Dec 2013. It has been a little more than a year and I am happy to report that both my knees are doing well. Dr. John Jaureguito is truly a good orthopedic surgeon. It took me a while to find a doc that I liked and who was willing to do both knees at the same time. If you have the will, it can be done — 1 hospital stay, going under once, pain med once etc … a 2 for 1! Plus allowed me to go back to work sooner! The follow-​up care by Dr. J was excellent. He truly cares about you and your well being. I highly recommend him.

Dr. John Jaureguito is truly a top notch orthopedic Dr. and surgeon. I have had several surgeries done him. He has a very good bedside manner and is an extremely competent surgeon. I do highly recommend to friends and family the services of Dr. Jaureguito. He is an asset to the community of Fremont.
Also the staff at F.O.R.M. (Fremont Orthopedic and Rehabilitative Medicine) are really very good at what they do. They are friendly and professional and make you feel very good about your tough decision of getting work done.
With the surgery center right there, the great medical staff and the great office staff you can’t go wrong with F.O.R.M.

As I’m coming to the end of my care with Dr Jaureguito l I want to take a moment to comment about how satisfied I am with everything about my service.
Although I’m sure my situation wasn’t anything out of the norm for them, it has surely been an experience for me. The results of which were a partial knee replacement as well as back treatment. I must say that everyone at FORM was without a doubt a pleasure to have met and dealt with throughout this process.
One of the nice things about FORM is that everything is all in the same building from Dr’s offices, x-​rays, MRI, PT to the surgical center.
Following a car accident on top of a pre-​existing condition much time, care and attention to my comfort was all a big part of my experience. It is truly all about quality of life when you’re talking to the doctors here.
I first met with Dr Dhanoa who treated me for my whip lash then referred me to Dr Jaureguito for my knee. All precautionary measures were taken prior to the decision to proceed with knee replacement surgery. Dr J always takes his time to discuss all options and potential outcomes. He listened to my feelings and took them into consideration. Dr J is always professional and respectful of his patients and his staff.
The staff at FORM is top notch. They are friendly and highly competent. It would be remiss of me not to say that there wasn’t a single person in the office that didn’t seem happy. I apologize I can’t name them all, just believe when I say everyone. That speaks volumes. This includes the PT department and Surgical center. So totally impressed.
Prior to the accident I was treated by a dr in Hayward. His office personnel were disengaging appeared to be less than interested in patients and really not a place I ever really wanted to go back to. I called them right after the accident to try to see the dr, just to see if anything had changed. It was less than 3 months since I’d been treated by his office. I did explain that there was a car accident. After the receptionists hesitation she clearly stated the dr doesn’t bull 3rd party. That was the end of that. I guess I should consider myself lucky :)
They have worked so well with me regarding the accident. I certainly didn’t realize how bothersome dealing with insurance companies, car insurance companies, lawyers and the variances when an accident happens in another state. I truly appreciate all they have done for me.

What do I say??? Dr. J is one of the kindest people I know. I have spent the last 30 months assisting first my sister with a single knee replacement, my brother with a hip replacement, my husband with double knee replacements and my dear friend with a single knee replacement. All done by Dr. J. He is kind, listens, encouraging, answers your questions and knows that each person is different. My father has a really bad knee but because of his age Dr. J has patiently been able to treat him for several years without surgery. HOWEVER THE most important thing is that if you need surgery he is an AMAZING SURGEON! He is with you from the minute the surgery begins to the minute HE closes and you are off to recovery. He and his staff walk with you through rehab and each person I assisted has their life back. His office staff is wonderful, they are a true team. I am so blessed to know Dr. Jaureguito and his entire staff.

The best doctor in the bay area. Dr. Jaureguito or Dr. J for short, has what many doctors lack, which is integrity, confidence and an outstanding practice with a caring staff and great professionalism. I had shoulder surgery today, I’m now home with no pain and confident on Dr. J’s work.
He never pushed for surgery, he exhausted every possible non surgical method of recovery, before we BOTH decided that surgery was the way to go.

IF you want the best go to Dr Jaureguito.
I had rotator cuff surgery on June 24th 2010 I had no pain after surgery. Dr jay is wonderful, he will explain everything in detail.
You’ll never feel rushed like some doctors, his bed side manner is outstanding. I consulted with a couple of other doctors before seeing Dr Jay. and was not impressed with their confidence or should I say lack of.
I’m having the other shoulder done as soon as I go though therapy for this one. Thank you, Tina Shepherd
