Dr. David Mikulencak attended Baylor College of Dentistry, earning his D.D.S. Following dental school, he completed a General Practice Residency in Indianapolis before starting his 30-​month orthodontic residency at St. Louis University, where he was President of his class. His thesis on expansion with the Damon System has been published by the AJODO. Dr. Mikulencak has been included annually in «Fort Worth Magazine» August Issue of Top Orthodontists since 2007. Selected for inclusion annually in «Texas Monthly’s» Super Dentists List since 2008. He is a Past Director Under 10 years for the Fort Worth District Dental Society in 2011 – 2012.
