If you have to file an insurance claim, GOOD LUCK! My insurance company tells me they have sent 3 requests to Dr. Heaven for additional information with no response to the first 2. I called Dr. Heaven after I received notice of this and he claimed the 3rd request was the only one he had received and he was responding that day. Today, I received an e-​mail from Cigna saying that they had received the Doctor’s response BLANK! Someone is lying.
